What happens at a pain management clinic during a psychological test?
Posted by admin in Clinic Management on March 2, 2012
Question by mylife: What happens at a pain management clinic during a psychological test?
I have chronic pain due to an accident in 2004. After 2 surgeries,(and another in two weeks), physical therapy, and treatment through my general practitioner, I have been refered to a pain management clinic. My first appointment left me hopeful. They scheduled me for routine psychological testing appointment that they do for all patients they are going to take as patients. I have never had testing of this sort. What does it entail and what should I expect?

You Have Nothing To Worry About.
A Psychological Examination Is Routine For Many Pain Management Clinics Due To The Fact That You Can Make Any Part Of Your Body Hurt By Simply Thinking It Enough Or Stressing.
They Will Evaluate Stress, and mental status so they will know what type of pain to treat you for exactly

Well it depends on the source of the pain, and whether it can eventually be resolved or not. One of the things they use a lot especially with back pain is an electronic stimulator. In the case of intractable pain, they may use a pain pump that utilizes morphine or other strong drugs, but they stay away from that in all but the worst of cases like crushed vertebra or other extreme circumstances.

Re: Herniated Disks And Sciatic Pain?
Posted by admin in Careers in PT on February 13, 2010
I was diagnosed with herniated disks in 1996. I had MRIs done in 1997 showing the herniated disks, I have three – two cervical and one lumbar. I was also diagnosed in 2003 with scoliosis. That doctor stated if my condition continues to deteriorate, I would be a candidate for surgery. This was in New York-I don’t live there anymore.
I moved to another state. I just had MRIs done and they don’t see any disk material protruding onto anything. They are saying the disk is dried out and that it is minimal.
I am in severe pain in my left leg. They sent me to physical therapy which only puts me in greater pain. I knew this but I told them I would do it their way because they didn’t believe how much pain I am in. I was out of it in pain for a week from that theraphy session and I haven’t went back for any more. I was in the ER on June 2 because I couldn’t stand upright from the pain in my back. That lasted a whole week of pain. I am tired of feeling so old.
I have tried ALL OTHER OPTIONS, I now am able to consider surgery as I have had this condition 12 years without improvement. I have gained over 130 lbs due to the inability to walk without being in severe pain. I have fallen down from the pain. I tried everything. I need assistance in hygiene sometimes because of the pain.
Where can I turn to? I have no insurance. I have tried to apply for social security disability, they turned me down because these doctors are saying there is no problem. The social security doctor said it’s obvious but they didn’t use his information in determining my eligibility. That doctor was an orthopedic, while I’m at the clinic seeing this menace of a doctor who should not be practicing but due to my insurance situation I had to deal with.
I’m going to another hospital because they actually have an orthopedic clinic there but it is going to take a while to get the approval for the indigent coverage.
I may be just venting, but I’m sick and tired of this crap. Please let me know if you know other options that I may have failed to realize. Thank you.
Serious College Athlete Been Out For A Year With Groin Pain, Just Want Answers!?
Posted by admin in Careers in PT on February 12, 2010
My name is Taylor and im a college soccer player and i’ve been hurt with a groin problem since November 4th 2007. Recently ive been assigned by a doctor to go to physical therapy about a groin issue which no doctor can really find, ive been through 2 mri’s, ct, and xray. But with all exams negative and me sitting out of soccer for over a year i’ve researched over and over on the internet and ive done some self evaluating and seemed to locate the pain in my left adductor with pain in left testicle and left lower adomenal/scrotum. The pain all started when i was running hills and overexerted myself and ended up vomiting. The next day i had significant pain and could hardly sleep at nights cause of the dull aching pain.No doctor or surgeon seems to really listen to me really about what i have to say which is understandable after the extensive years of school they’ve been through. Once the injury happened i went to my school clinic and they immediately labeled me as having a sports hernia and sent me to surgeon and such, surgeons would just do the cough method on me and say i just have a strained groin and give it time. I believed them and gave it time and after about 7 straight months of resting just a “strained abdomimen” and still feeling the same pain while coughing, and sudden movements actually probably even worse since my groin seemed to be tighter because i hadn’t exercised, (besides the bike or olypitcal at low speed to stay in somewhat decent shape.)I decided to just start exercising running playing soccer every now and then, and i could feel my groin, the pain was there but once it got loose it felt okay but mid way through it’d start to get bad, especially after and was sensitive and sore for a couple days, but since the numberous doctors and surgeons turned my away i didn’t have much to lose. I eventually stopped cause i couldn’t take the aching pain, however it still has never went away even when im not exercising for an exenstive period of time.Anyhow i was assigned 4 weeks of therapy 3 days a week and i was told if my groin problem didn’t get any better from physical therapy (which it hasn’t, and im in my last week of therapy) then they are going to perform surgery where they are going to go in and just search for the problem and hope they find it and can fix it supposebly. Im a big sports fan so i’ve known about you from hearing about athletes needing sports hernia surgery. From what i noticed not many doctors believe in a sports hernia so they don’t even really give it any chance. Anyhow i thought i would send you an email and see what you had to say about it, it’s probably hard to make evaluations over the internet so i understand, sorry this is all crammed together and thank you for time, i just am interested in what you think about the situation. Coaches are frustrated with me, all i want to do is play soccer again.Thank you again.
Severe Pain?
Posted by admin in Careers in PT on February 10, 2010
I have had severe pain in my right side for over 4 years. I am currently on disability due to severe pain. I was approved the first time. I have been to multitudes of doctors, I’ve been to the Mayo Clinic, and I may go back again. I’ve had all kinds of tests, and they really haven’t had any answers. They give me narcotics and they send me to the pain clinic. I’ve been to the pain clinic many many times, and the pain clinic doesn’t have any answers for me either. Mayo found that I had 4 severely rotated vertebrae, and I was referred to a Chiropractor to untwist my spine. I went to Physical Therapy and completed 17 months of physical therapy. My pain level is better but still severe. I was referred to the Mayo Chronic Pain program but my insurance would not pay for it. I’ve tried several times. I’ve tried every possible thing I can think of. Any ideas? The doctors have told me that they have tried everything and cannot help me.
Thanks So Much !!
Chronic Lower Back Pain Tried Everything, Ideas?
Posted by admin in Careers in PT on February 9, 2010
My husband has chronic lower back pain as a result of an injury many years ago. MRI shows nothing other than slight fluid in the facet joints. We have tried physical therapy (which made it worse), chiropractic, multiple anti-inflmmatory pills, steriod injection into the back, burning the nerves, pain management clinic, Vicadin, and various opiate based drugs. it is so debilitating he can’t work- a simple walk through the grocery store is almost more than he can handle and he has a pretty high pain tolerance. Sitting is also uncomfortable. He can’t hold our baby unless he lays down because he is afraid of dropping him if a spasm hits. This has been going on for a year now and has had a major negative impact on our lives. Has anyone else dealt with a similar problem and did you ever get a diagnosis and treatment that worked?
What Can I Do For Wrist Pain?
Posted by admin in Careers in PT on February 8, 2010
I have been having pretty significant joint and wrist pain. I saw my dr last week and she gave me a referal to Physical Therapy. I have an appointment on Tues. I asked if she could prescribe something for the pain and she gave me a referal to the Pain Clinic, meanwhile Im suffering in a lot of pain. Tylenol 3 helps most of my other joint pain but not my wrists. And nothing helps the wrist pain aside from bengay type of ointments and they only work for maybe half an hour. There has to be SOMETHING? Anyone?
Nerve Pain?
Posted by admin in Careers in PT on February 7, 2010
I’ve had severe nerve pain for about 4 years, I am on disability – I was approved the first time. That should tell you how severe it has been. T11 and T12 were severely rotated, and T10, L1 L2 were also rotated. The pain runs across the #11 Nerve – Intercostal nerve. They tried meds like Neurontin and Lyrica, they helped with the pain a lot but I couldn’t tolerate either, both caused me severe side effects and I had to quit both. All they can do is give me narcotics and send me to the pain clinic – both methods are ineffective for me.I’ve improved a lot in the past year, mostly through physical therapy – but I still don’t have enough control of pain – and I’m trying to get where I can work again. My wife died a year ago (she was 42, I am 45 now) -and I don’t want to be home all the time,I want to be productive, and I try very hard. The pain along the 11 nerve is still fairly bad,and if I go off the narcotics it’s unmanageable. I’ve gone to lots of doctors for 4 years, not much help
What Are Some Good Alternative Medical Therapies For Back And Neck Pain?
Posted by admin in Careers in PT on February 6, 2010
I have a herniated disc, a bulge in another part of my back or neck, my neck is straighter than it’s supposed to be. My back and neck muscles are all knotted up effecting my motor skills in my legs, weakness in my limbs, I have pain that spreads from my lower back and wraps around my abdomen.
I’ve done physical therapy twice, what they call manipulation, just of full body PT massage, I’ve gone to a chriopractor. I have an apoint at the med rehab clinic in a month. My doctor said I may possibly have to get a cortosone shot. What are some other options? I heard they can put the disc back in place, or surgery. I’m scared about all 3 because of the pain. Has anyone tried massage therapy or accupuncture? Do I have any other options?
Can Getting An Epidural Lead To Back Pain Later On After Child Birth?
Posted by admin in Careers in PT on February 5, 2010
I’ve been having chronic moderate to severe back pain for around 10 months now. After many ER and Doctor visits, I’ve had almost no answers except one Doctor who mentioned it sounded like it may be from the epidural, which my pain didn’t begin until almost a year after having my daughter. They sent me to a pain clinic who then sent me to do physical therapy. The physical therapist noticed I had scoliosis which I wasn’t aware of because all I have ever been told was that I had a slight curvature of the spine. No one has directly told me I had scoliosis. I know they asked me if I had it before I got my epidural and I said no of course…Anyway I guess the pain may have took awhile because of scar tissue building up and maybe messing with some nerves..but my mid to lower spine just aches as well as having some fiery pain directly to the right of my spine in that same area. It’s worse when the weather is gloomy and the only thing that feels good is to bend over and arch my back and let my arms dangle. I’d appreciate any comments or suggestions…
